Thanks,
I think the idea has potential but think you need some examples (pictures) and more than just a image and form. make up some bogus requests and show the results (but I didnt tell you to do that )
That image btw should be text, you'd get a lot more chance of being picked up by a search engine if you did, it's also a little fuzzy (text).
When I see things like this, I always wonder why I have to tell you my email address, what purpose would you need it for? and every time the only thing I can come up with is spam, so maybe you should add a comment or something about why the email address and even add a privacy policy (if you have one).
If I was you I'd change the form your using, the form mail script not only shows your email address for all to see it's also prime pickings for spam bots (harvesters) and in some cases kiddie hackers (are able to query the form for your details), use server side script if you can (php/asp)

I really do agree with everything Busy has said on this one and apart from the fuzzyness of the text as a graphic you aren't going to get search engine's crawling the site as they wont see anything! Also light grey text on white is hard to read, stick with black.
There isn't really much else to critique as there isn't a design/layout as such although the logo could possibly be bigger and maybe a different unique font.
Good luck with it

Do not end sentences with a preposition.
It is sufficient to say that you will shoot "anything that visitors request" without adding "for" (you write, "anything that visitors request for".)
Just a tip from an English teacher.

There really isn't much to critique here other than you shouldn't use a graphic for your main text. Is this just a temporary page?
I didn't submit any info so I don't know what the other pages look like (if there are any).
