Information Storage

hi,

im looking for a script (after looking for it on search engines/resourceindex.com) which allows me to give visitors fields to fill in;

NAME:
TEL:
EMAIL ADD:

(and anymore/less fields i can add myself) so that when the member fills it in it is stored in a db somewhere, i dont need no hi-tech admin panel or anything ,just something that stores information.

im gonna continue to look for this script but hopefully someone can help me here.
