I used to just use notepad and hate on everything else. Basically now I use only DW for mass creating sites. Even then I hand code everything in source view.

hand coding isn't a dieing breed, a lot of webdevelopers use an editor but most that do server side as well usually hand code.
I still hand code, don't know what I would do if I couldn't have my editpad

I think it's more efficient to use a combination. For my site I use a combination of a cms (blog) script and hand coding. In other cases I might use a combination of Dreamweaver and hand coding. I love dreamweaver for this because it makes it so easy to switch back and forth. Why should I waste my time writing paragraph and list tags when DW can do it just as well? I do hand-code all my CSS. I'm faster in CSS than any editor.
