Is this on your home computer or your web hosts?
If its your home, and you just installed mysql etc then username would be root and password would be empty.
If it's your web host then you need to set up the database through your control panel (ensim, plesk etc) and this will assign the name and password for you (that you choose)
phpmyadmin is just an editor,
take a look at http://www.netadmintools.com/art90.html if you want to reset the home pc password

I think you can always apply to your hosting support for help.
