Help with database issue

hello,

I run a website for a Virtual Airline that I run for flight sim 2004. We are looking for a way to make an interactive flight board... I have some knowledge of MySQL but still learning a lot. The question is, how would we create an interactive script so that we can post our flights on the board and have our pilots put their name or initials in there to show that they have taken that flight.. That is the only way that I know how to put it lol...

After the flight, display a form (html form) so the pilots/user can add name/initials/password (ideally should be name and password so cant be done for other people) and then this information from the form just inserted into database.
Basically it would be a login form with information stored in database rather than cookie
