Can you please explain exactly what you mean? Are you looking for a pre-built database containing all this information, or are you asking how you create a database to do this for you?
Why not just use a standard dropdown?

www.oscommerce.com has all that data already in its database but I'm not sure if the license extends to using just that data...
I would have though it would as its open source but even so...

No I checked, that database doesn't contain city names, just province/state and country.
