I'm trying to setup a very simple trouble ticket system. I have a basic formal form asking some questions. Towards the bottom I want to user to select what type of problem they have (ie. Sales, Support, Other). Depending on what they select, I want the contents of the form e-mailed to a different address (ie. [email protected], [email protected]). How ould this be accomplished?

Secondly, I want the subject automatically inserted as "Support Request 1000", "Support Request 1001", and so on. This should change each time a new ticket is opened. In simple terms how would that be accomplsihed?
