Help please

Guys I need some help. I run Hugeimage.com it is free image hosting. Anyways the problem I'm having is that the actual upload field does not show to my users once they login and try to upload a file. I recently switched servers and this problem occurred. To see what I'm talking about in more detail go to hugeimage.com/

I don't know what's wrong with the coding or if it's a server problem but I am requesting someone's professional knowledge to get it back working. Please forward any comments on this to me or if you need more information. Thank you all in advance for your help in the matter.

Since you're using Javascript to open up the "add more fields" category, and it doesn't...i would say one of two things. Either the path to the JS file is wrong, or coded incorrectly (possibly got corrupted?).

It's server related not an error in the php I think I figured it out. Thanks for everyone who took a look.
