CGI script for HTML generator? - Where can I find one?

Hello,

I need to find a CGI script that would take the answers the user would provide in a form and generate a table (or chart) incorporating the input - for instance, creating a customized plan.

Also, I'd like to know if such a script will work in an "offline" environment - like an ebook - that's not connected to the Internet.
