your best plan is to start with a collocation. once you have enough fundage to afford it get a t-1 with a few routers and set up more boxes. then pray that people stay with you and the even more come. once you've done that rent a whole office building and setup cool rooms, security and other cool tools

Usually ISPs buy from bigger ISPs and resell.
The companies on top have leased lines between their network centers and "the backbone".
The backbone is a network of big (well, still to small) pipes between really big companies, universities and government agencies.
If you never want to pay "anyone" again, just setup your own network.
If you find a hub every 3-4 miles you can go wireless.
