Thats one hard code to read, very hard to read, one thing i did find extra, the submit button doesnt use a name value option, but the reset button isnt working either so its something in there.
If no one has replied by tonight (my time) I'll go a huntin
but you can check things yourself, make sure every value has two double quotes, that all tags that are started are closed <> , that there are no extra < or >'s in there.
IF you wanted to, you could remove all those font tags that =3 (and there end tags, they arent needed, one at the top and closed at the bottom is all you need if your not using style sheets.

The codeshould be better to read now.
The problem with span lang and font tags are eliminated by using css now on our pages. And it was while making this page too into using css we found out that the form doesn't work anymore.
We might find another form out there that has been made for free but this one has been good to us all the time it worked. Don't know when it stopped funtioning. It worked in october 2001.

Easy one.
You closed your form tag too early. The submit button (as well as many other tags) are outside of the form
