"tell a friend" popup script

Hi-

I'm looking for a simple script I can put on my main page
that has a link that says "tell a friend about this site" and when
they click on it a window appears that has 4 boxes;

Your name:
Your email:
Your friends email:
Comments:

and then they can quickly send a message to their friends email/

Anyone know of one?
