Poll Help

I am looking for a poll script, an online poll service, or anything, that can let me run a poll which specifies a required number of choices to pick, eg. Pick 2 from the following. Most polls either allow you to pick one, or an unlimited numbero of answers, can anyone guide me on how to get a poll that lets you pick a required number of answers.
