What you will need is a server side script, using something like PHP, ASP, PERL, etc.
Which one really depends on what languages your hosting company uses.
As a last resort, if you don't have scripting languages available on your hosting, you can try someplace like http://www.freedback.com/features.php jsut keep in mind that then you are opening form contents up to 3rd parties, so you don't want to ask for information like this client of theirs does: http://www.myoverheadsupport.com/ that is a major security risk there.
