Help with form (and please hurry!)

Hello ~ I'm running a monthly drawing on my site: http://2kcut.com/drawing.shtml

In the form code, I'd like to require the email address of the entrant. Here's the piece of code that REQUIRES their email address:

<INPUT TYPE="Hidden" NAME="required" VALUE="Your email is required.">
<INPUT TYPE="Text" NAME="from" SIZE="40" MAXLENGTH="100"><br>

It seemed to work fine when I tested it, but a customer just brought it to my attention that it won't let you submit even when you DO enter the email address.

Please tell me what I've done wrong. I'm very new at this stuff, and I honestly thought I had it working right.

In addition to the "required" problem, I have another question:

When I originally set this up, the email I received showed the "from" name ~ I don't know exactly how to explain this. Anyway, now all the "entries" show NO sender's name on the email I receive.

If I knew what I had done to make it go away, I would fix it myself.
