Create a Membership Form Using Javascript

Javascript is the dynamic, attractive and best option to create a table. I have recently launched a website (I am not using any CMS) and want to add a membership form in the right corner of my website. I hired a freelancer but he totally messed up my website. In this position i decided to do it myself. Can anybody suggest me some tutorial or some coding that i can create a membership form by myself.
